For many NDIS participants spending time in hospital, the lead-up to Christmas can feel especially challenging. Finding a safe and supportive home before the holiday season can make all the difference—bringing comfort, stability, and peace of mind heading into the new year.
At Thrive, we work closely with key hospitals across NSW to create discharge pathways that support participants to return to the community. Our goal is to ensure people can enjoy Christmas outside of the hospital while making long-term plans for their future.
With the support of our dedicated clinical team, we make the transition process as smooth and supportive as possible. Over the years, we have assisted dozens of participants to move into transitional Medium Term Accommodation (MTA) while helping them get everything in place to eventually find their forever home.

We will actively support you to connect with our network of accommodation providers, ensuring you have a range of options when finding your forever home. At Thrive, we understand that moving from hospital to home is more than a transition—it’s a fresh start. With the right supports in place, participants can leave hospital with confidence and begin the next chapter of their lives.Â
Accomodation
A range of options located close to major hospitals, providing up to 90 days of Medium Term Accommodation following discharge.
Community Support
Strong partnerships with allied health providers, ensuring therapists are available within 7 days of discharge to complete reports, assessments, and equipment requirements.
House Setup
Fully furnished accommodation options with short-term rental equipment available, reducing stress during the transition period.
Fast Discharge
With the support of our clinical nursing team, we can shorten admission times and facilitate safe discharges within 7 days.
