Who Is Thrive365?

Thrive365 was launched in 2017 with the long-term vision of providing exceptional at-home care that is tailored with Participant’s to ensure needs are meet and that they are empowered to be their best selves.

We consider ourselves as a small-medium size company that we feel is our strength in providing our participants with responsive and a centred-person approach of care. We provide people living in private, shared NDIS housing accommodation across Western Sydney and South- East Queensland. We pride ourselves on our friendly approachable customer service, our staff are consistent, well trained, and have access to continuous learning and professional development. Our residents choose to live in our supported homes long term because they feel safe, feel at home, and are supported to live their lives on their own terms. 

Reimagining SIL

We founded Thrive365 to create a radically different way of experiencing “home”. We know first-hand how disability housing has worked in the past, and it’s abysmal. Accessible houses are not the only ingredient needed to create accessible homes. The places we create are not just “housing”. We help our residents to live their best home. Shelter is one of everyone’s basic needs, one of the things we all need to survive.

But we’re not interested in just meeting basic needs with NDIS housing. People with disabilities have been forced to just survive for too long. We help our residents to thrive. Residents in our homes have meaningful choice and control in their lives, the ability to take their own risks, express fun and joy. They have the space to grow into a more authentic version of themselves. We use the Person Centred Active Support framework.

How we Work With Participants

Implementing a Person Centered Approach. 

A person-centred approach is where the person is placed at the centre of the service and treated as a person. Our focus is on the person and what they can do, not just on their condition or disability. Support should focus on achieving the person’s aspirations, be tailored to their needs and unique circumstances. 



Supports our Participant, at the ‘center of the service’. To be involved in making decisions about their life.


Getting to know each Participant’s life experience, age, gender, culture, heritage, language, beliefs and identity.


Participants are experts in their life, we focus on what they can do first, & assist where they need as a second.


We always include the person’s support networks as partners.

The Thrive365 Team

At Thrive365 we pride ourselves on creating a friendly, diverse and professional workplace culture, we understand by having a strong culture it then flows onto how we interact with our Participants for improved experiences and better outcomes.

Our staff have the necessary qualifications and are supported to have continuous opportunities for training and upskilling. 

We can provide a complex level of care in the following: Peg feeding, Chronic obstructive pulmonary disease management, medication management, wound care, Diabetes Management and Tracheostomy care. 

Our support staff are provided guidance from our Health Liaison team that have experience in aged care, disability care and health care. We also have by appointment the availability of an RN nurse for complex care support.

We carefully build our teams around Participants with qualified support workers who have access to ongoing training, access to guidance and up to date best practices through the support of our Health Liaison team. 

Our teams work locally every step of the Participant journey before a suited home is found & then transitioned into. We work supporting Participants and their team of supporters from onboarding, to finally moving in and creating an environment where Participants can start living their best lives.
